Thank you for taking the time to fill out this form! If your project idea has been selected you will be required to submit a non-refundable $250 deposit to solidify your appointment date. The deposit will be taken off the price of the tattoo at the end of the appointment and is equal to 1 hour of tattoo time. PLEASE BE MINDFUL of scams or anyone trying to impersonate this booking system. There will be an in depth response to your inquiry with steps on how to make a deposit connected to this form it will only be coming from stephaniespeertattoo@gmail.com.

Multiple ideas are welcome within the same application under “Design Concepts & Ideas” please be sure to number them in order of priority.

Deposit payments can be made through e-transfer or if needed can be made through a credit card purchase through the shop upon request and subject to a 3% fee.
