Thank you for taking the time to fill out this form! If your project idea has been selected you will be required to submit a non-refundable $250 deposit to solidify your appointment date. The deposit will be taken off the price of the tattoo at the end of the appointment and is equal to 1 hour of tattoo time.
Multiple ideas are welcome within the same application under “Design Concepts & Ideas” please be sure to number them in order of priority.
This is a zero tolerance space for disrespect of any kind. We are all human and doing our best despite high demand. Failure to comply will result in permanent client termination - no exceptions.
Deposit payments can be made through e-transfer or if needed can be made through a credit card purchase through the shop upon request and subject to a 3% fee.
